INSURANCE REIMBURSEMENT
If you have insurance, different copayments and deductibles are required by various group coverage plans. Your copayment is based on the specific policy selected by your employer or purchased by you. The practice will do everything possible to determine coverage and expected fees (e.g., deductibles, out-of-pocket, copays) prior to starting services. However, this may not always be possible due to many challenges, such as insurance back-logs, technical difficulties, or missing/delayed insurance information. It is very important for potential you to determine what mental health services are covered by their insurance plan. Most claims will be sent directly to insurance companies, unless it is determined that Headway, our billing partner, will be more effective in processing specific claims. Typical insurance claim response time is about 10-15 business days. It is possible that your insurance company may respond with a non-coverage or non-payment response, which will become the responsibility of the client to pay.
You are responsible for notifying the practice immediately of any changes to your insurance. If you fail to notify the practice of any changes to insurance, you may be billed for services that are not covered. You will be responsible for payment of all charges.

Cancellations Procedures and Fees
At Insight Therapy, we understand that life happens. To ensure we provide the best care and maintain availability for all clients, we have established the following policies:
Cancellations and Rescheduling
Notice Requirement: Please provide at least 24 hours advance notice to cancel or reschedule an appointment, excluding emergencies. Failure to provide adequate notice will result in a $125.00 fee assessed to your account.
Sliding Scale/Open Path Clients: The late cancellation/no-show fee is $55 for clients under a sliding scale or Open Path option.
No-Show Policy
As a courtesy, your provider may call or text you within the first 15 minutes of your scheduled appointment if you have not arrived. If you do not respond or are unable to join, the appointment will be considered a no-show, and the $125.00 fee (or $55 for sliding scale/Open Path clients) will apply.
Important Note: If the courtesy call or text is not made by your provider, this does not exempt you from the no-show fee.
Missed Appointments
If you miss or no-show for 3 consecutive appointments, your provider will discuss whether therapy is the appropriate fit for your needs at this time.
If the practice is unable to contact you or you fail to reschedule for 30 days, your file will be considered closed due to inactivity.
